About Hair

Hair Structure
Hair is composed of a durable protein called keratin. A hair follicle anchors each hair into the skin.

The hair bulb forms the base of the hair follicle. In the hair bulb, living cells divide and grow to build the hair shaft. Blood vessels nourish the cells in the hair bulb, and deliver hormones that modify hair growth and structure at different times of life.

Hair growth occurs in cycles consisting of three phases:

At any one time, about 90% of the hair on a person's scalp is growing. Each follicle has its own life cycle that can be influenced by age, disease, and a wide variety of other factors. Hair grows at different rates in different people; the average rate is around 1-2 cm per month. The average adult head has about 100,000 to 150,000 hairs and loses up to 100 of them a day.

Types of Hair Loss

  • Pattern hair loss (male or female) vs non-pattern hair loss
  • Actual hair loss versus hair breakage
  • Focal hair loss versus diffuse hair loss
  • Hair thinning versus hair shedding
  • Scarring hair loss versus non-scarring hair loss
  • Hair shaft miniaturization versus reduced density

Causes of hair loss:

1) Hereditary hair loss (Androgenic Alopecia)
The most common cause of hair loss is a hereditary condition called male-pattern baldness or female-pattern baldness. It can inherited from either paternal or maternal side of the family. It may or may not skip generations. It is caused by combination of genetics, hormonal changes and the aging process. Among all, genetics is the most important factor contributing to one’s hair loss.

2) Medical conditions
Alopecia areata – It usually causes patchy hair loss. It occurs when the body's immune system attacks hair follicles, causing sudden hair loss that leaves smooth, roundish bald patches on the skin. This condition may result in complete baldness (alopecia totalis). But in about 90% of people with the condition, the hair returns within a few years.

Some skin illnesses – Certain skin diseases that can cause damage to hair follicles by inflammatory reaction may result in permanent hair loss at the scarred areas. These conditions include lichen planus, discoid lupus, sarcoidosis etc. Besides

Scalp infections – Infections, such as ringworm, can invade the hair and skin of the scalp, leading to scaly patches and hair loss. Once infections are treated, hair generally grows back

Other conditions – Other types of systemic problems such as low thyroid hormone, iron deficiency, collagen disorder, growth or sex hormone deficiency, secondary syphilis all may cause diffuse hair thinning

3) Telogen effluvium
It is the temporary hair thinning over the scalp that occurs because of changes in the growth cycle of hair. A large number of hairs enter the resting phase at the same time, causing hair shedding and subsequent thinning. It is usually stress related hair loss. It usually occurs about three months after the precipitating event. Examples of trigger events include

  • sudden excessive weight loss
  • high fever or acute illness
  • surgery or general anaesthesia
  • emotional stress
  • drug induced (Oral contraceptive pills, blood pressure medicines, anti-cholesterol agents, cimetidine, coumarin, Vitamin A etc)

4) Anagen Effluvium
This refers to sudden hair loss due to the effect of chemicals or radiations such as hair loss following chemotherapy, radiation therapy of toxins. Hair loss begins 1-3 weeks following exposure to the inducing factor. It could be temporary and hair growth is back to normal once chemotherapy treatment is finished.

5) Certain hairstyles or hair treatment
Excessive hairstyling or hairstyles that pull your hair tight, such as pigtails or cornrows, can cause traction alopecia. Hot oil hair treatments and permanents can cause inflammation of hair follicles that leads to hair loss. If scarring occurs, hair loss could be permanent.

6) Hair-pulling disorder.
This condition, also called trichotillomania, causes people to have an irresistible urge to pull out their hair, whether it's from the scalp, the eyebrows or other areas of the body.

Alopecia Areata

Pattern Hair Loss (Male and Female)

Pattern hair loss or Androgenic alopecia is strongly linked to following factors:

Actually both men and women produce “male” hormones. The most important hormone that has effect of hair growth or fall is called dihydrotestosterone (DHT). In people who are genetically susceptible to balding, the exposure of certain part of hair follicles to DHT over long period can cause male and female pattern baldness.

This hormone makes the susceptible hair follicles in those regions of the scalp (over the front, top, and crown of their head) produce shorter and finer hair (“miniaturization”) over time. Ultimately these follicles will eventually stop producing hair shafts.

The balding process can occur at any age and progress at any rate. It can start in the teen years (as early as early 20) or late in life; develop rapidly, slowly, or even stabilize.

Hair Transplant

Hair transplants are minor dermatologic surgical procedure in which hair follicles are transferred from the permanent and thick donor area around the sides and back of the head to areas of thinning or balding generally found on the front, top, and crown of the head, as well as eyebrows and beard areas.

Hair transplant is performed as an out patients surgery under twilight local anaesthetic. The procedure usually last 6-8 hours depend on the amount of grafts being transplanted. Modern techniques of hair transplant can restore lost hair and replace or re-shape your hairline with your own natural, growing hair, which need no more care than the ordinary washing, styling, and trimming you have always done.

Benefits of Hair Transplant

There are many types of hair loss (also called alopecia). It can be classified into few ways:
Increased self-confidence and self-esteem
